The Third Annual Local Take Community Health Forum takes place on Saturday, December 5 from 2:00 p.m. until 6:00 p.m. at Northlake Mall, 4800 Briarcliff Road, NE Atlanta, GA 30345. I'll be your host for an afternoon-long public event designed to help families achieve a healthier 2016.
The Office of Congressman Hank Johnson is bringing representatives from Social Security, Medicare, Medicaid and the Veteran's Administration to provide information and answer questions. The 100 Black Women of DeKalb County will provide mental health screenings and childhood obesity prevention information.
In addition, Oakhurst Medical will lead our ASK THE DOCTOR PANEL and provide diabetes and high blood pressure screenings. LifeSouth Blood Services will be on hand to take your blood donations and, provide information on the Bone Marrow Registry and Cord Blood Services.
We will have navigators on hand to answer questions about the Affordable Care Act and sign up individuals for coverage for 2016.
